Being a client or model for Melissa Glynn Photography, I grant permission for select images mentioned below to be used in print or digital form on the photographer's blog, website, social media accounts (including Pinterest, Instagram, Facebook, and Twitter), printed sample items for studio, advertisements and promotions. (Include all image numbers you agree to releasing. If you're releasing the full collection, please write "Full Collection" and the date of the shoot below.

I, for good and valuable consideration, the receipt of which is hereby acknowledged, hereby irrevocably authorize Melissa Glynn Photography to use photographs of me and or my property and authorize her and her assignees, licensees, legal representatives and transferees to use and publish (with or without my name, company name, or with a fictitious name) photographs, pictures, portraits or images herein described in any and all forms and media and in all manners including composite images or distorted representations, and the purposes of publicity, illustration, commercial art, advertising, publishing (including publishing in electronic form on digital storage or internet websites), for any product or services, or other lawful uses as may be determined by the photographer or studio named here. I further waive any and all rights to review or approve any uses of the images, any written copy or finished product. I am of full legal age and have read and fully understand the terms of this release. If at any time I would like to revoke the use of my images by the photographer, I understand that I am responsible to pay the photographer’s full rate for all items I received in exchange for the use of my images. $100 per high resolution digital image delivered.
